Dental Authorization Form

If your pet is scheduled for a dental procedure, filling this out form will explain to us what procedures you are willing to authorize, as well as contact information so we can contact you, if needed, during the procedure.  Once your pet under anesthesia, a complete oral exam is performed. It is common that previously hidden problems are found at this time.  Letting us know how involved you wish us to get, and how to contact you if needed, can save critical time while your pet is anesthetized.

CANINE Pre-Anesthetic Blood Work 

If your dog is scheduled for surgery, we strongly recommend that a pre-anesthetic blood profile be performed.  Blood chemistry testing provides an inside look at your dog’s vital organs, such as the liver and kidneys.  The function of the liver and kidneys is especially important during surgical procedures because these organs process and rid the body of medications used during anesthesia.

A pre-anesthetic profile does not guarantee the absence of anesthetic complications; however, it may greatly reduce the risk of complications as well as identify medical conditions that could require treatment in the future.

Even if you would like to opt out of the blood panel, we will still need this form filled out.
